At the end of an aorto bifemoral bypass case the patient has 4 twitches with no fade and sustained tetany with tidal volumes of 250 mL, should the patient be reversed?
The short answer is no. In this case many clinicians would give reversal mostly for medicolegal reasons. However, a good clinician will ask, "What will the drug do to the patient(e.g. neostigmine will induce peristalsis)and do the benefits outweigh the risks when considering the type of procedure they had (e.g. Not ideal if patient has had GI surgery).
